* We require a credit card on file for payment of services. Your financial information is kept confidential and secure as per industry standards for medical and financial information.
Payment Terms
1. Payments are due at the time of or prior to your appointment date and time.
2. Your card on file will be charged prior to your appointment. Our staff may run your card on the day of service, as early as 8 am.
3. You may pay your balance on the invoice prior to your appointment directly through our portal.
4. If your credit card is expired or does not have sufficient balance, your appointment may be canceled.

Other Fee Disclosures
* Forms Fee: $50.00-$100.00, depending on the complexity of the information required.
* Clinical Summary and Letters: Fees vary from $100 - $300, based on the complexity and time required.
* Returned Check Fee: $35
* Prescription Refill Fee: May be charged for patients who request refills soon after missing an appointment.
Agreement and Payment Arrangements Your appointment may not be confirmed or may be canceled if the agreement is not signed and payment arrangements are not made.
You agree not to dispute the payment with credit card company as long as the transaction corresponds to the terms indicated in this policy. If there is an error or a charge I do not understand, I will inform Frederick Psychiatry to resolve any inconsistencies.
